Caring for Your Expander Appliance

  • What is an Expander?

    A rapid maxillary expander (RME) is an orthodontic appliance that widens the upper jaw by gently pushing the upper palate wider, correcting a narrow palate, which can cause dental crowding and crossbites.

    Expanders are typically used in children whose upper jaws are still growing. The device is adjusted by turning a key daily. The expansion phase lasts a few weeks, after which it remains in place for several more months to stabilize the expansion

  • We'll do the prep

    One week before installing the RME, we will place separators – rubber bands flossed between your teeth – that will stay there for a week, creating enough space to seat the RME comfortably.

  • How will it feel?

    While turning the RME, you may notice slight pressure on the roof of your mouth or even tingling in your nose.

    Over-the-counter pain relievers can help manage this temporary discomfort. If these sensations remain difficult to manage, Dr. Paul may instruct you to turn less frequently and complete the prescribed expansion over a longer period.

  • What's next?

    After expansion, orthodontic tooth movement will be used to bring the front four teeth back together, perfecting your smile!

How to: Turn the RME Key

You can do it!

Dr. Paul and your orthodontic assistant will show you and your child how to turn the key on your expander. Sometimes it can take doing it yourself a few times to feel confident, but we promise, it’s simple. Here’s how:

  • Tip the head back.

  • Have someone help shine a light in the mouth so you can see better.

  • Use the key provided to rotate the expansion screw per Dr. Paul’s instructions. If you lose the key, we have extras!

  • Push the key toward the back of the mouth until it stops and you can see the next hole.

  • Carefully remove the key from the mouth. You should be able to clearly see the new hole for the next turn.

Care & Maintenance

  • Brushing and Flossing

    Brush and floss daily.

    Brush gently in a circular motion.

    Make sure to get the outer, inner, and chewing surfaces of each tooth. Don’t forget to clean your gums!

    Brush for 4 minutes (instead of the usual 2 minutes) to ensure all surfaces are clean and healthy.

    Brush after snacks and meals to remove any leftover food.

    Continue to see your general dentist for regular cleanings throughout treatment.

  • Loose or Broken Appliance?

    Loose or broken expanders can be uncomfortable — call the clinic immediately if these issues arise, and we’ll determine the urgency with you and schedule a comfort appointment or make a note for your next visit. These visits are expected from time to time and are included in your treatment plan at no additional cost to you.

    Avoid hard or sticky foods which can pull the expander off your teeth.
